    Abstract

    BACKGROUND: 

    Ischemic heart disease (IHD) is a leading cause of death in India. Dietary changes could reduce risk, but few studies have addressed the association between diet and IHD risk in India.

    OBJECTIVE: 

    The goal was to address the association between diet and IHD risk among Indians in New Delhi (northern India) and Bangalore (southern India).

    DESIGN: 

    We collected data from 350 cases of acute myocardial infarction and 700 controls matched on the basis of age, sex, and hospital as part of a hospital-based case-control study in 8 hospitals. Long-term dietary intake was assessed by using food-frequency questionnaires developed for New Delhi and Bangalore. We used conditional logistic regression to control for the matching factors and other predictors of risk.

    RESULTS: 

    We observed a significant and dose-dependent inverse association between vegetable intake and IHD risk. The inverse association was stronger for green leafy vegetables; in multivariate analysis, persons consuming a median of 3.5 servings/wk had a 67% lower relative risk (RR: 0.33; 95% CI: 0.17, 0.64; P for trend = 0.0001) than did those consuming 0.5 servings/wk. Controlling for other dietary covariates did not alter the association. Cereal intake was also associated with a lower risk. Use of mustard oil, which is rich in alpha-linolenic acid, was associated with a lower risk than was use of sunflower oil [for use in cooking: RR: 0.49 (95% CI: 0.24, 0.99); for use in frying, RR: 0.29 (95% CI: 0.13, 0.64)].

    CONCLUSION: 

    Diets rich in vegetables and use of mustard oil could contribute to the lower risk of IHD among Indians.

    Safe steps in food handling, cooking, and storage can prevent foodborne illness. There are four basic steps to food safety at home:

    • Clean - always wash your fruits and vegetables, hands, counters, and cooking utensils.
    • Separate - keep raw foods to themselves. Germs can spread from one food to another.

    Nutrients (food substances for survival & growth of organism) that are not manufactured as such in the human body, e.g. Niacin, vit. C and otherwise essential for good health are known as Essential Nutrients. 
    Some categories of essential nutrients include vitaminsdietary mineralsessential fatty acids, and essential amino acids. Different species have very different essential nutrients. For example, most mammals synthesize their own ascorbic acid, and it is therefore not considered an essential nutrient for such species. It is, however, an essential nutrient for human beings, who require external sources of ascorbic acid (known as Vitamin C in the context of nutrition).

    Many essential nutrients are toxic in large doses (see hypervitaminosis or the nutrient pages themselves below). Some can be taken in amounts larger than required in a typical diet, with no apparent ill effects. Linus Pauling said of vitamin B3 (either niacin or niacinamide): "What astonished me was the very low toxicity of a substance that has such very great physiological power. A little pinch, 5 mg, every day, is enough to keep a person from dying of pellagra, but it is so lacking in toxicity that ten thousand times as much can [sometimes] be taken without harm. NIACIN ( 1500mg ) daily may prevent STROKE in CV patient and may be prescribed in Dyslipidemia and type 2 Diabetes.

    PROTEINS: Amino-acids are building blocks of proteins. Histidine, Arginine ( half essential), Lysine, Leucine,
    Valine, Isoleucine, Methionine, Phenylalanine, Tyrosine, Threonine (HALLVIMPTT) are essential amino acids.
    Egg protein ( white of egg )- a good protein like that of milk, has 100 as its biological value ( B V ).
    Vegetable Proteins are of low B.V.
    D-Aspartate Acid ( an endogenous amino acid ) is pro ( sexual) hormone. It is also related to BRAIN FUNCTION.
    CEREALS ARE mostly d efficient in Lysine and pulses in Methinine.
    Bread is enriched with LYSINE. Combination of Pulses and Cereals in the DIET may yield optimum benefit.
    L-Carnitine another non-essential amino acid is related to energy production.
    Soys ( 40% protein), Peanut ( 25% ), pulses ( 15% ), Meat, poultry, Fish and Egg-white ( 8-15% ),
    Cereals ( 7-12% ), Human Milk ( 1.2% ), Cattle Milk (3-8% ) and most vegetables are 1-2% protein.
    ALL ENZYMES ARE PROTEINS. These regulate body functions e.g. ARMIDASE ( present in abdominal wall, incriminated
    in OBESITY. Converts Testosterone to Estrogen ( a female hormone ). It's inhibitors are in the market. )
    Thyroid Hormone is protein based ( Tyrosine+ Iodine ). Blood cells and serum are loaded with protein.
    Provide ENERGY. Its deficiency may lead to Protein Energy Malnutrition ( PEM ).
    Thus proteins are responsible for body mass and function.

    FATS: Linoleic acid (LA, 18:2) an omega -6 fat and Alpha Linolenic Acid (ALA, 18:3)- an omega 3-fat respectively are essential fatty acids ( EFA ). 
    Arachidonic acid (AA) is a long chain omege-6 and DHA and EPA both are long chain
    omega-3 fatty acids. 
    Omega-3 to omega-6 ratio in the diet should be 1:<5.

    MILK contains CONJUGATED LA - a healthy fatty acid.
    OLIVE OIL IS RICH IN MONOUNSATURATED FATS e.g. Oleic Acid-a cisfat. Olideic Acid is a TRANSFAT bad for health.
    So also HYDROGENATED FATS OR REFINED/ overheated oil. 
    Mostly Sesame, Olive Oil, MUSTARD OIL and CANOLA OIL are good for cuisine.

    In a 70 kg Reference Male bone mass is about 5 Kg. mostly minerals= calcium & phosphorous.

    The prophet (SAWS)
    regarded fig, dates and pomegranate as fruits from paradise. He encouraged eating of carrots , fruits and consumption
    of the black (cumin) seeds, barley water and use of MISWAAK (brushing & flossing of teeth).

    He (SAWS) used to eat watermelon with fresh dates and the
    tharid ( ثريد ), which is composed of bread and meat. He ate cheese and dried dates with bread or alone.

    It may be added here that lipids and
    milk fats consist of CLA- a beneficial Fatty acid/lipid However butterfat/ghee should be consumed sparingly as it contains Saturated fats too.

    Honey ( almost 100% sugar ) blended with cold
    water may also be consumed . Diabetics to consume a teaspoonful only. 

    Water is essential for life. (koran 21/30). The pH ( a measure of acidity & alkalinity ) of blood is alkaline ( 7.35-7.45 ). So is that of potable drinking water. 
    Prefer foods with moisture. The pH of fresh milk is also alkaline. 
    Don't eat while reclining or
    drink water respiring in the vase and wash hands before and after eating.
    <EAT AND DRINK: BUT WASTE NOT BY EXCESS>. Koran 7/31.
    Further Koran says, <We made from water every living thing>. Koran 21/30.

    CitrusFruits e.g. lime, lemon- too acidic. ( AVOID THEM !) in general interfere with drug absorption and inhibit enzyme cytochrome p 450 and may contribute to hyperacidity of stomach.

    More than 2 billion people are at the risk of Iodine Deficiency Disorders ( IDD ) all over the world,
    in spite of the fact that their prevention is relatively easy and inexpensive.
    In INDIA alone > 70 million people are affected by IDD clinically. Sub-clinical ( covert/hidden ) Hypothyroidism is an ice-berg phenomenon surpassing its overt/clinical outcome in its occurrence.
    Goiter ( enlargement of thyroid gland ) is endemic in most countries of the world. Only one country Japan is virtually free from IDD where sea-weeds and fish food consumed in plenty.

    A middle aged adult male of a medium built complained of chest pain. He also complains of lethargy, easy fatigue,
    loss of concentration and anxiety. An year ago he had a brief spell of loss of consciousness. At times he experiences ' misbeats'
    ( Arrythmia ! ).
    On physical examination: The patient has a protruded belly- abdominal girth is 2 inches more
    than that of chest. BMI 28 (slightly over-weight ). Slow speech. No thyroid enlargement.
    Relishes all sort of foods.Both systolic and diastolic blood pressure and pulse rate slightly raised ( tachycardia !).
    X-ray chest normal. ECG suggests ischemia. Blood picture shows micro-cytic anemia.
    Thyroid Profile: T3 & T4 slightly reduced and TSH increased. Slightly raised fasting blood sugar level. There is increase in serum VLDL cholesterol level. His elder sister died of "heart attack". Thyroid disease not ruled out.
    DISCUSSION : When one eats too many carbohydrate rich foods, the pancreas secretes insulin to move excess glucose from the blood into the cells where glucose is used to produce
    energy. But over time, the cells lose the ability to respond to insulin. It’s as if insulin is knocking on the door, but the cells can’t hear it. The pancreas responds by pumping out even more insulin in an effort to get glucose into the cells, and this eventually causes insulin resistance leading to hyperglycemia.
    Studies have shown that the repeated insulin surges common in insulin resistance lead to thyroid gland destruction. As such thyroid hormone production falls. In this case there is hyperglycemia,
    although hypoglycemia is common in hypothyroidism.
    Dys-glycemia ( hypo/hyper-glycemia ) is a feature of abnormal thyroid states ( dys-thyroid ).
    It should be noted here that hypothyroidism itself precedes high serum tri-glycerides and cholesterol levels. However co-morbidity with Diabetes and Heart-artery disease and obesity independently cannot be ruled out. Clinical findings highly
    suggestive of METABOLIC SYNDROME.
    MYXOEDEMA (the term almost inter-changeable with severe hypothyroidism ) i.e. deposition of connective tissue in and under skin refers to non pitting oedema of htpothyroidism but may occur in hyperthyroidism also. A ( THYROID )crisis may follow loss of consciousness and may prove fatal.
    ADVICE: The patient is advised to continue thyroid hormone therapy. Other family members to be screened for Thyroid disorders. Periodical testing for hormone levels required. Cardiologist and diabetologist to be consulted.
    Physical activity ( 30 minutes' brisk walking everyday ) is a must.
    Health ( education ) measures at family or individual level : Avoid over-eating. Prefer diet rich in protein and low in fat ( but high in omega3 fats ) and carbohydrates, minerals and fiber.

    MEASURES at COMMUNITY LEVEL: Improve, promote and legislate Applied Nutrition Programs ( AWARENESS & AVAILABILITY of BALANCED DIET ) for vulnerable groups ( pre-school, school children, pregnant women, lactating mothers and seniors ) at various levels.Ban production, sale & consumption of SOFT DRINKS. Promote the sale of milk/low fat milk.

    FOODS/DRINKS to be AVOIDED or LIMITED in general are: 1) High fat foods. foods too rich in saturated fats/ omega 6 fats:- Almond, Cashew, Coconut (both grains/nuts and oil), and Safflower, corn and sunflower oils , and animal foods (which also contain Cholesterol) such as dark meat, organ meat, brain, butter fats, yellow of eggs. 2) Drinks with too much sugar or refined sugars/bakery items in general/cola/Pepsi or any other soft drink. 3) Rice deficient in protein & calcium. 4) Foods interfering with Iodine metabolism (Goitrogens i.e. causing Goiter ):- Cabbages, Broccoli,Okra ( Bhendi ), Cauliflower. ridged gourd ( Turai ), Turnip, Radish and peanuts.
    Citrus fruits/juice and Citric acid Myth. There is a common notion/ myth that citrus fruits restore vitality. Hence  its usage in patients or well clients alike. However here is an evidence based warning against this practice.The citrus fruits contain natural chemicals called furanocoumarines beside citric acid which is more acidic than vinegar ( citric acid ). Concomitant consumption of citrus fruits/juice in patients on certain medications may result in adverse side effects Persons on drugs fighting infections, bad- cholesterol reducing medicines, conception control pills/shots, on hormone replacement therapy and E.D. medications are specially advised not to consume citrus fruit/juice/soft drinks.     FOODS to be PREFERRED in general are: 1) Protein rich foods, low fat milk/curd ( rich in water, quality proteins & minerals) and cow's milk. 2) White of egg, Fish, lean meat, Black and Green gram dhal ( rich in protein & other nutrients. 3) Methi seeds, Walnuts, Fat fish (rich in omega 3 fats) 4) Green leafy vegetables, Carrots, French beans, Onion, Soybeans, Pumpkin seeds, Berries ( straw, blue and kiwifruit), Apple, Melons and other seasonal fruits and vegetables, 5) An egg a day is recommended for senior citizens, children, pregnant and lactating mothers. 6) Iodized salt, honey & vinegar in small amounts. Honey ( in small amounts ) is an excellent remedy specially in various forms of ALLERGY. 7) Sprouts. 8) Alpha-amylase rich foods & protein rich Laddus made out pulses & sugar/ jaggery for children. 9) Mustard oil as coking medium. 10) Water: About 2 L/day ( requirement for healthy adults ) in addition to the moisture present in ordinary foods. Consume water on par with the amount of the required food or the physical activity in relation to the environment.
    11) Iodized salt to be spayed on or added to cooked meals.

    IODINE together with TYROSINE ( an essential protein unit ) in the THYROID gland forms the hormone.
    Iodine deficiency in the diet leads to thyroid disorders through its hormone levels in the body.
    Iodine a lustrous solid or violet -blue gas, is essential and precious to humans so much so that about a mere 100 mcg/day/individual stand between human intelligence and imbecility.
    What iron is to the blood or calcium and phosphorous to the bone, IODINE is to the thyroid- a butterfly shaped gland. IODINE is attracted to the THYROID as iron-fillings to a magnet. Thyroid is a relatively vascular organ, comprising two lobes joined by an isthmus and lying just anterior and slightly attached to the cartilages of the larynx.
    Iodine ( the stable form or non-radio-iodine/I 127 ) and one of its isotopes 131 iodine were discovered in 1811 and 1931 respectively.
    When Almighty created life on earth with water/soil, the trace element iodine was given its reign to rule the body metabolism and also perchance to be responsible for its annihilation through one or two of its unstable forms ( isotopes ) namely radio-iodine ( I 131 & I 129 ).
    The dietary IODINE ( the organic form e.g. iodides/iodates ) in the body, concentrated in the thyroid in combination with TYROSINE- an essential amino acid - is part of the hormone that controls the body metabolism ( including growth and development ) as and where energy is produced and spent including body temperature mechanism. Thus it acts like blowing the smouldering fire ( of life ).
    On the other hand the radio-iodine when released in the atmosphere/ environment competes with the stable iodine and may render the thyroid inoperative and halt the energy metabolism. The other Iodine isotopes with smaller half-life namely I 123, 124 and I 125 have some limited role in Nuclear Medicine e.g. the treatment of thyroid cancer, as the x-ray contrast media. I 131 is present in nuclear fission products. Chernboyle disaster and Japanese nuclear accidents of late are some of the manifestations of its vast destructive effects.     (IDD) namely Goiter, Cretinism/ Congenital Hypothyroidism are common, more in females than males and due to DIETARY IODINE DEFICIENCY which in turn is related to iodine deficiency in the soil and water. Deccan plateau ( TELANGANA ) in India is no exception.
    Accelerated deforesting, soil erosion,recurrent flooding, multiple cropping with widespread use of alkaline fertilizers, bacterial load of water, poor sanitation and other forms of malnutrition due to poverty, ignorance and infection and infestations further add to the enormity of this problem.
    The IDD may result in irreversible brain damage in the fetus and infant, affect stature in childhood 
    and retard psycho-motor development in the child. Iodine deficiency is the most common cause
    of preventable mental retardation. It also affects reproductive functions both in male and females. Hypothyroidism may also result following thyroid/neck surgery. 
    Certain compounds present in vegetables (Goitrogens) specially brassica species may prevent
    iodine uptake in the body and cause Goiter. Excessive calcium in water may also be responsible for goiter endemicity.
    The common goitrogens are : cabbage, beet root, cauliflower, ridged gourd and soybean.
    Marine foods (e.g.) Fish contain IODINE. Consumption of IODIZED SALT prevents IDD.
    IDD are currently a significant public health problem in 118 countries.It affects females more than males. 
    Iodine deficiency disorders (IDD) are endemic in India and Nepal.
    Iodine in the body is required to produce Thyroid hormones T4 and T3. In sub-clinical hypo-thyroidism Thyroid stimulating hormone ( TSH ) level may be increased without any significant decrease in T3 and T4 levels.

    Reverse T3 dominance ( WILSON's SYNDROME ), is marked by increased prolonged levels of rT3, where conversion from T4 to T3 is blocked.

    Also bulging eyes, weight loss. low blood cholesterol and sugar may be apparent.
    It is mostly an autoimmune disease, known as Grave's Disease with a fatal outcome.
    It is an immune disease where auto-antibodies get attached to thyroid stimulating hormone receptor cells and continually stimulate it.
    It may also occur due to infection or over-dosage of thyroid hormone treatment in hypothyroidism. 
    Anti-thyroid drugs may cause blood disorders. One of the major complications in thyroid surgery ( as in Goiter removal also ) is injury to the nerves in the neck region which may affect speech.
    THYROID CANCER is uncommon and may occur even in normal thyroid states ( eu-thyroid ).
    It is treated with radio-active iodine or surgical resection.
    THYROID STORM is an emergency and refers to UNTREATED hyperthyroidism.

    CALCIUM in the DIET
    Besides IODINE, Calcium, Phosphorus and vitamin D3 are also involved in bone formation and 
    affect stature/body height. Both calcium deficiency specially or its excess may cause 
    kidney stones. Phytates/Phytic acid in (wheat) bran 7 in SOFT DRINKS, tea and coffee prevent calcium absorption in the gut while vitamin D facilitates its absorption.
    One study suggests that calcium intake with Vitamin D3 is associated with a lower risk of total cancer and cancers of the digestive system, especially colo-rectal cancer.
    Hyper-calcaemia (high calcium levels in blood) which in itself be secondary to bone
    resorption/and low Ca diet (hypoparathyroidism), may lead to widespread organ dysfunction and damage. 
    It may cause ' painful bones, kidney stones, abdominal groans and psychic moans'. 
    Hypo-calcemia/calcium deficiency is rampant in our country affecting specially 
    children, elderly and woman folk. It is a major cause of short stature, osteoporosis,
    kidney stones ( strangely! ) and dental problems affecting children. 
    Mild Ca deficiency may cause Tetany i.e. muscle cramps and paralysis of respiratory muscles with difficulty in deglutition. 
    Severe form may result in laryngo-spasm and death. 
    Rice is deficient in calcium and iron. Ragi- a millet, is rich in both. A direct source 
    of calcium is calcium oxide (lime) that is applied to betel leaves (pan), the chewing
    of which is a popular custom among INDIANS. Dairy products are its good source.
    The absorption of calcium is helped by lactose (milk sugar), lysine and arginine.
    Calcium supplements are advised for osteoporosis specially in women. However
    their dose should be minimal as these supplements without adequate Vitamin D3 may
    cause Cancer of Pancreas, heart and artery problems .

      Saturated fats enhance the production of cholesterol & bile acid which are tumor promoters and are responsible for increased prevalence of colo-rectal cancer. Coke and other soft drinks prevent the absorption of dietary calcium. Avoid soft drinks of all kids. A recent study incriminates these as the cause of pancreatic cancer apart from others.
